In order to take part in the 2013 symposium you must register. You may register either online or via mail. When you register online, you will be sent an acknowledgment via email confirming receipt of your registration. We urge you to register as early as possible to ensure your participation. Seating is on a first come, first served basis. Please plan to arrive 30 minutes before the program starts.This educational event is expected to sell out, and space is limited.
An *early bird registration fee of $60.00 for online / $75.00 for mail-in / fax registration before Friday, February 15, 2013 and $75.00 after that date is required to attend this event, which includes lunch and parking.

A form of payment must be included in order to process registration form.
Requests for refunds made at least five days before the start of the class will be granted. A $10 administrative fee will be charged. We reserve the right to cancel any class within five days prior to the class start date with a full refund to participants. We reserve the right to make program changes in the event of speaker illness or natural or unforeseen disaster. No refunds will be made if the registrant does not attend the conference.
